Legal name
The name as it appears on your tax return — for a sole proprietor, this is your personal legal name; for an LLC or corporation, your registered business name.
Tax classification
Your business structure for US tax purposes: individual/sole proprietor, C corporation, S corporation, partnership, trust/estate, or LLC (and if LLC, which of the above it's taxed as). This should match what you filed with the IRS when you formed the entity.
TIN type — SSN or EIN
Your Taxpayer Identification Number is either a Social Security Number (common for sole proprietors operating under their own name) or an Employer Identification Number (common for LLCs, corporations and partnerships). Use whichever one is tied to the legal name you entered above.
Address
Your current mailing address — street, city, state and ZIP — matching the address on file with the IRS where possible.
Certification
A declaration, under penalty of perjury, that the TIN you provided is correct, that you are not subject to backup withholding, and that you are a US person. This is a legal certification, not a formality — it must be accurate.
